Step 2. Create Username and Password

ON THE NEXT PAGE, CREATE A USERNAME AND PASSWORD FOR YOUR ACCOUNT, THEN CLICK THE GREEN REGISTER BUTTON (AS SHOWN IN THE IMAGE BELOW).

Step 3. Update Profile and Continue to Application or Active Provider Contact Update Form

FINALLY, REVIEW YOUR PROFILE DETAILS ON THE PROFILE PAGE, CHOSE YOUR CONTACT PREFERENCES AND THEN CLICK THE GREEN “UPDATE” BUTTON TO CONTINUE TO THE JOB APPLICATION OR ACTIVE PROVIDER PORTAL UPDATE FORM.
